When your loved one is considering end-of-life care, the ability to spend those days at home provides additional comfort at a very difficult time. Surrounded by the things they love most, patients receiving hospice care at home find a safe haven where they may live with dignity and comfort.
Halcyon Hospice staff members provide support to the primary caregiver, typically a family member or loved one who may have never cared for a terminally ill person. Our team develops a care plan designed specifically for each patient’s need for pain management and symptom control, as well as their emotional and spiritual needs.
Halcyon Hospice will also provide medication, treatment, equipment and supplies that are related to the patient’s hospice diagnosis. A member of the Halcyon Hospice team makes regular visits to assess the patient and works closely with the patient and family to provide additional care or services when needed, all in the home setting.
Halcyon Hospice staff is on call 24 hours a day to answer questions and provide assistance. We provide services, based on patient and medical needs, seven days a week, 365 days a year. Anyone can make a referral, including patients, physicians or family members.
